Medical (military medical) commission

       The Medical (military-medical) commission is a structural subdivision of the State Institution "Territorial Medical Association of the Ministry of Internal Affairs of Ukraine in Kharkiv region" (hereinafter - M(M)C), which was established to conduct a military-medical examination to determine the suitability of candidates for service in the National Police and the National Guard, candidates for admission to higher educational institutions of the Ministry of Internal Affairs, the suitability of police officers and servicemen in their career transfer, dismissal.

       Heads of the M(MM)C Commission is doctor Kostiantyn Viktorovych Zubkov.

      The main tasks of the M(MM)C are:

  • Determination of the suitability of candidates for service in the police, candidates for admission to higher educational institutions of the Ministry of Internal Affairs with specific training conditions that train police officers (hereinafter - educational institutions), applicants for the higher education of educational institutions, suitability of police officers for further service during promotion, service on a business trip abroad.
  • Determining the health and physical development of candidates for military service under a contract, in the military reserve, candidates for admission to higher military educational institutions, and the degree of fitness of servicemen for military service in connection with their dismissal.
  • Determination of the causal relationship of diseases, wounds, injuries, contusions and mutilations in police officers in service, dismissed from service and dismissed from service, servicemen of the NGU, former servicemen, former rank and file and senior officers of the internal affairs bodies (hereinafter - former rank and file and senior officers).
  • Determination of the causal relationship of diseases, wounds, injuries, contusions and mutilations that caused the death of police officers, servicemen, former police officers and servicemen, former rank and file and senior officers.        

            List of medical documents for persons entering the service of the National Police of Ukraine and higher educational institutions of the National Police of Ukraine:

  • Referral from the personnel department for medical examination in the form (Annex 4).
  • Registration certificate or military ID for men, passport for women, 1 3x4 photo (for study),
  • Graduates of educational institutions must submit a medical characteristic.
  • A detailed extract from the medical record of the medical institution at the place of residence, work, service or study for the last 5 years (the extract must be certified by the doctor's signature and the seal of the medical institution).
  • Certificate (certificate) of psychiatrist and narcologist (from the place of registration).
  • Certificate of mandatory preventive vaccinations (form 063/o) in 2 copies.

       Note: Former military personnel must have a medical characteristic, a copy of the conclusion of the M(MM)C, and a copy of the extract from the order of dismissal.

       Candidates for police service must provide documented results of the above examinations conducted within one month before the medical examination at the M(MM)C (according to paragraph 10. p. II of the Order of the Ministry of Internal Affairs of Ukraine dated 03.04.2017 №285). 

       Candidates for training must provide documented results of these examinations carried out within one month before the medical examination in the Migration Service of Ukraine (according to paragraph 10. Section II of the Order of the Ministry of Internal Affairs of Ukraine dated 03.04.2017 №285)

       

       List of medical documents for persons entering military service under a contract to the National Guard of Ukraine and for studying in higher educational institutions of the National Guard of Ukraine:

  • Referral from the commander of the unit for medical examination according to the form (Annex 14).
  • Registration certificate or military ID card for men, passport for women,
  • Registration certificate or military ID card for men, passport for women, 1 3x4 photo (for study),
  • Graduates of educational institutions must submit a medical characteristic.
  • A detailed extract from the medical record of the medical institution at the place of residence, work, service, or study for the last 5 years (the extract must be certified by the doctor's signature and the seal of the medical institution).
  • Certificate (certificate) of psychiatrist and narcologist (from the place of registration).
  • Certificate of mandatory preventive vaccinations (form 063/o) in 2 copies.
  • Certificate from the tuberculosis dispensary.

       Note: Former servicemen must have a medical characteristic, a copy of the conclusion of the M(MM)C, and a copy of the extract from the order of dismissal from service. Candidates for military service under the contract must provide documented results of the above examinations conducted within one month before the medical examination at the M(MM)C.
